After "All that remains", Carver seeks out Hawke. Words are exchanged, tempers flare more than usual, old grudges get dragged up, and then they start fighting for real. Eventually, Carver manages to get Hawke/Hawke's magic pinned and since he's hurting so badly and wants "dear older sister" to feel even a fraction of it, noncons her. After the deed or towards the end, it hits Carver about exactly what he's done. Bonus if part of the reason Carver can get away with it is because on some level, Hawke still blames herself and feels like she deserves it.
Play up the horror for this one without fetishizing the noncon, and emphasize that Templar!Carver is treating Mage!Marian exactly how the worst of the Kirkwall templars treat the mages in their charge (with the implication being that his new "friends" are rubbing off on him), and he likely goes so far as to threaten her with the Rite of Tranquility.
And then descend into the horror of the guilt. (Depending on Marian's level of self-loathing/blame, possibly some mindbreak on her part, as well.)
For a really bad ending, include the companions' reactions to what happened--I imagine Fenris and Anders particularly would both massacre Carver, Fenris because he's twitchy enough all the time (he killed his cowering/begging sister, ffs) and Anders because 1) he's obsessed with Hawke & 2) it fits in perfectly with his/Justice's vendetta. Which isn't to say Merrill, Isabella, Aveline, and Varric wouldn't fuck shit up for her honor--though Sebastian's lack of an appropriate reaction might be interesting, too.
And if I don't want the revenge ending, there's always the more subtle horror of Carver and Marian coming to some kind of unspoken agreement to pretend it never happened.
